Ask questions you can act on
Before sending the survey, explain who can read responses, whether answers are identifiable and how long they will be kept. Do not describe a survey as anonymous merely because it omits a name: combinations of role, location and work schedule can identify someone in a small team.
Use this ten-question template
Copy the questions below into your approved survey tool and adjust the available options to what you can realistically provide. Allow people to skip questions and include 'none' or 'prefer not to say' where appropriate.
Suggested introduction: 'We are considering optional workplace wellbeing activities and want to understand what would fit your workday. You may skip any question. Please do not include medical or identifying details in free-text answers. We will explain how responses are handled before collecting them.'
Turn answers into a schedule
Compare interest in each format with feasible attendance windows. Preserve the time zone attached to every local time; 12:30 in New York and 12:30 in Los Angeles are different opportunities. Check actual calendar dates when daylight-saving changes affect teams.
A popular preference is not a reservation. Confirm coverage and instructor availability before announcing a class. Where there is little overlap, compare repeated sessions or another format rather than assuming everyone can attend one lunchtime.
Close the feedback loop
Tell employees what you learned and what decision it informed, without sharing identifiable responses. Explain when the next check-in will happen. Keep a separate voluntary contact step if people want follow-up, instead of automatically attaching their email to an otherwise anonymous survey.
The template is available without signing up. BarreWorkout’s employer interest form is a separate company inquiry; it is not an employee survey and it does collect the contact email and company details shown on that form.
